Introduction

Exercise Treadmill Test (ETT) & Cardiopulmonary Exercise Test (CPET)

Centre Medical offers two specialized stress tests—Exercise Treadmill Test (ETT) and Cardiopulmonary Exercise Test (CPET)—to comprehensively assess how your heart and lungs respond to physical exertion. These tests help doctors make accurate diagnoses, evaluate treatment outcomes, and tailor personalized exercise or rehabilitation plans.

Exercise Treadmill Test (ETT)

Also known as a Cardiac Stress Test, ETT uses a treadmill to simulate physical exertion while continuously monitoring your electrocardiogram (ECG) to assess how your heart functions under stress.

Purpose:

  • Detect the risk of coronary artery disease (e.g. myocardial ischemia)
  • Identify exercise-induced arrhythmias, chest pain, or shortness of breath
  • Evaluate the effectiveness of treatments (e.g. medications or angioplasty)
  • Provide a functional baseline before surgery or cardiac rehab

Who Should Consider This Test:

  • Individuals experiencing chest pain, shortness of breath, palpitations, or dizziness
  • Those with cardiovascular risk factors such as hypertension, high cholesterol, or diabetes
  • Patients undergoing or recovering from cardiac treatments
  • Anyone seeking a personalized exercise or rehabilitation plan

Preparation & Risks:

  • Wear comfortable clothing and running shoes
  • Follow doctor’s instructions regarding medications
  • Not suitable for patients with recent heart attack, severe arrhythmias, or mobility limitations
  • Though non-invasive, rare complications include arrhythmias, heart attack, or fainting (risk ~1 in 2,500)[Stuart & Ellestad, 1980]
Advantages

Cardiopulmonary Exercise Test (CPET)

CPET is an advanced stress test that measures oxygen uptake (VO₂ max), carbon dioxide output, and overall cardiopulmonary performance using gas exchange analysis during exercise. It provides objective data on your body’s metabolic and functional capacity.

Purpose:

  • Evaluate cardiopulmonary fitness and VO₂ max
  • Guide personalized exercise or rehabilitation programs
  • Differentiate the cause of shortness of breath (cardiac vs pulmonary)
  • Pre-operative risk stratification (e.g. valve replacement, transplant)
  • Assess functional limitations in chronic diseases like heart failure or COPD

Who Should Consider This Test:

  • Individuals with low fitness levels or suspected cardiopulmonary impairment
  • Patients preparing for or recovering from heart surgery
  • Those with heart failure, COPD, pulmonary hypertension, obesity, or Long COVID
  • Professional or amateur athletes seeking baseline performance assessment

Preparation & Risks:

  • Test involves wearing a face mask and ECG leads, and performing moderate-to-high intensity exercise
  • Avoid large meals, coffee, or tea before the test; wear athletic attire
  • Temporary symptoms may include fatigue, dizziness, or shortness of breath; medical staff will closely monitor
  • Generally low-risk, but rare arrhythmias or discomfort may occur

※ CPET is available exclusively at Centre Medical Cardiac & Neuro
